"Human Error Prevention" Seminar:

The principles and practices of human error prevention are universally applicable regardless of the type of industrial, commercial or governmental enterprise, and regardless of the type of function performed within the enterprise.

This seminar is truly unique and up to date with the latest developments in human error prevention. Marguglio's taxonomy of human error causal factors and his human error-related models demonstrate his leadership in this subject. Examples and case studies amply reinforce the human error prevention principles and practices.

This seminar covers: * Classifications of human error; * The four levels of human error; * The three levels of barriers to human error and to the hazards activated by human error; * The four types of barriers at each barrier level; * Methods by which to make barriers effective; * Error-inducing conditions; * Behaviors to counteract error-inducing conditions; * Thought processes and behaviors leading to non-conservative decisions; * Thought processes and behaviors leading to conservative decisions; * Techniques to prevent recurrence of error - e.g., coaching; * Metrics; * Defense in depth.

"Root Cause Analysis" Seminar:

Marguglio's analytical logic and rigor, with unique analytical templates, yield greater consistency in the application of the various root cause analysis tools and greater effectiveness in the results of root cause analysis.

Upon completion of this seminar, one will be able to: * Understand and use problem identification, problem reporting, data codification, data grouping, data analysis, extent of condition, causal factor analysis, and corrective action terminology; * Perform root cause analysis using a variety of techniques; * Design and implement a problem reporting, root cause analysis, and corrective action system; * Manage or assess such a system.

"Nuclear Procurement Quality Requirements for Design/Construction Services & Components" Seminar:

This seminar covers dozens of contractual clauses that make quality and economic sense for the procurement of design and construction services and components for both new facilities and major modifications to existing facilities.

Although the focus in this seminar is on contractual clauses that are applicable to nuclear powered electricity generating facilities and U.S. Department Of Energy nuclear laboratories, and although ANSI/ASME NQA-1 and ISO-9001 will be the basic source documents, quality and procurement engineers from other types of high technology, complex facilities (e.g., gas and oil facilities, ports, military facilities) would definitely benefit from this seminar because the clauses can equally apply to these facilities.

For quality and economic benefit, the contractual clauses go beyond the standards noted above. The implementation of a few of these clauses can have the potential for avoiding tens and hundreds of thousands of dollars in unnecessary expenditures.

The major elements of each clause will be described and the logic supporting the technical and economic benefits of the clause will be discussed.

The Commercial Grade Item (and Service) Dedication (CGID) process will be described, along with Failure Mode and Effects Analysis, it being a necessary element of CGID.

"QEH&S Auditing, ISO 19011" Seminar:

This seminar covers in detail the overall audit system and the process for planning and performing an individual audit. The differences in the process for internal audit, supplier audit and third party audit are addressed. The consolidation and integration of quality, environmental, health and safety audits (and security audit, as well) are addressed.

Upon completion of this seminar, you'll be able to: • Understand and use audit terminology; • Understand the differences among internal, supplier and third party auditing; • Understand how to integrate the processes for quality, environmental, health and safety (and security) auditing; •Understand the factors to be considered in establishing an audit system; • Understand and use a "compliance-based" or "effectiveness-based" audit objective; • Understand a "risk-based" or non-risk based audit scope; • Understand "system-based" / "program-based", "performance-based" and "results-based" data collection techniques; • Perform and lead audits; • Design, develop, implement and assess an audit system.
